Our agency is committed to serving and supporting individuals with intellectual and developmental disabilities
along with their families, irrespective of the spectrum of supports needed.

Supports may encompass a wide range of services, including but not limited to providing person-centered
assistance with community-based activities, accompanying and enabling individuals to attend events,
and individual supports - assisting in completing activities of daily living such as medical care,
household tasks, transportation, and finding suitable housing.

Additionally, our support extends to facilitating hobbies, recreational activities, peer socializing,
and addressing lifestyle needs tailored to each individual's care level.

BEHAVIORAL SUPPORTS

We assist individuals in managing challenging behaviors, so that they can experience more independence, safety, reduced stress and enhance their ability to actively participate in their communities.

CAREER PLANNING

We support serviced individuals in exploring, identifying, and achieving their desired career paths. The scope of this service covers individuals with IDD who are transitioning from school to work, seeking new employment opportunities, or aiming for career advancement.

- Skills Assessment: Identifying existing skills, strengths, and interests relevant to potential careers.

- Career Exploration: Introducing individuals to various career options aligned with their abilities and preferences.

COMMUNITY BASE SUPPORT
(One-to-One)

This encompasses a wide range of services designed to help serviced individuals live independently and participate actively in their communities.

COMMUNITY INCLUSION

We recognize that social interaction, meaningful connections, and access to community resources are vital for a fulfilling life.

Our Community Inclusion therefore focuses on empowering serviced individuals to participate actively in various aspects of their communities, thereby helping to combat social isolation and loneliness, while fostering a sense of belonging and connection.

INDIVIDUAL SUPPORTS (CCP)

Individual Supports (CCP) is a wide ranging service designed to address the unique needs of serviced individuals who are part of the Community Care Program (CCP).

RESPITE

Provides temporary relief for families and caregivers of individuals with Intellectual and Developmental Disabilities, thereby affording caregivers the opportunity to take a break from their caregiving responsibilities, and promoting well-being for both the caregiver and the individual in the process.

SUPPORT BROKERAGE

Encompasses a wide range of services designed to help serviced individuals live independently and participate actively in their communities. This service includes hiring and managing (Self-directed Employees (SDE), helping to locate housing, filling out paperwork etc.

SUPPORTED EMPLOYMENT
(Individual)

We help individuals with Intellectual and Developmental Disabilities find, secure, and maintain competitive integrated employment.

This essentially helps to foster independence, self-sufficiency, and social inclusion for the serviced individuals.

Providing ongoing support in the workplace in instances when the serviced individual has been employed.
